Washington County Employees Has Access To The Following Services For Pain Management

Acupuncture – 20 visits per calendar year.

Chiropractic – 20 visits per calendar year.

Trigger Point Injections, that may also incorporate aspects of the various injection techniques
which Dr. Hardy has studied over several years to get you feeling better faster.

Your health insurance through Select Health, will pay for these services by Dr. Hardy
for your $25.00 co-payment per treatment.

Utah requires car owners to carry  personal injury protection (PIP), which pays for the medical costs occurred with

your your auto accident.


Your PIP coverage will cover those bills up to $3,000.00, unless you have purchased a higher amount of PIP coverage,

no matter who was at fault in your accident.


Dr. Hardy will treat you for the amount of personal injury protection (PIP) that you have purchased which will pay for the majority of services offered in his office, related to pain management due to the injuries sustained in your auto accident. 


Dr. Hardy doesn’t accept liens or work with personal injury attorneys.

Health Insurance and Medicare

The majority of health insurances do not cover the services that Dr. Hardy provides in is office.

Dr. Hardy isn’t not a provider for any insurance companies, with the exception of Select Health.

Dr. Hardy can provide the service(s) to you with the option to pay at the time of your visit.

If you choose to do this, let the front office know at the beginning of your appointment, this way you can receive a

copy of your superbill to turn into your insurance company for reimbursement in case they do cover the services provided.

Regarding Medicare, the services that Dr. Hardy provides isn’t covered by Medicare with the exception of Chiropractic, which Dr. Hardy isn’t a provider for.
